From the Public Health Operations Center in Santa Cruz County:

If you are an administrator 
for a health care or other essential business or organization, complete the business survey below once on behalf of your organization so Santa Cruz County can plan for your needs and contact you if needed to schedule vaccination. 

If you are a Phase 1a healthcare worker and a patient of Kaiser, Sutter/PAMF, or Dignity Health, check their websites for any announcements about providing vaccine to their Phase 1a patients. If you have a different primary care provider, check our COVID vaccine web page at for information about Phase 1a clinics which are in development but not posted yet.

COVID-19 Vaccine Distribution Notice


Santa Cruz Community Health would like to help vaccinate Santa Cruz County dental practices. There will be COVID vaccine clinics open

Friday 1/15 (9 a.m. – 4:50 p.m.) 

Saturday 1/16 (9 a.m. – 4 p.m.)

Space is limited and sign-up is on a first-come, first-served basis. To sign up, please click the following link:

Please note that Santa Cruz Community Health is only vaccinating healthcare workers in Phase 1a as per Santa Cruz County and CDPH guidelines. This includes any individual currently working in a dental office. If the sign-up list is full, or you can not make any of the times available, check the link in several days as we are doing our best to increase capacity.

If sign-up lists continue to be full, and you are interested/able to potentially receive your vaccine on very last minute notice, you may email Debi Diaz at to get on a first-come, first-served standby list. Do not call, email request only.
