Soft and Hard Tissue Management Around Implants in the Esthetic Zone
Successful implant therapy in the esthetic zone depends as much on the tissues around the implant as on the implant itself. This two-hour program reviews the biologic and clinical factors that protect soft- and hard-tissue volume from extraction through restoration. Dr. Al Rezk will discuss how the transmucosal zone of the abutment influences the peri-implant soft-tissue seal, drawing on his published literature review in General Dentistry (2018) on soft-tissue response to abutment materials and surfaces, including titanium and zirconia. He will also present socket preservation in the esthetic zone, including the vascularized pedicle combination epithelial-subepithelial tissue graft technique he described in Clinical Case Reports (2019). Clinicians will leave with a practical framework for planning extractions, preserving ridge and soft-tissue architecture, and selecting abutment materials that support long-term peri-implant health and esthetics.
Learning objectives
At the conclusion of this course, participants will be able to:
- Describe the role of the peri-implant soft-tissue seal in protecting crestal bone and supporting esthetics.
- Compare how abutment materials and surface treatments (titanium vs. zirconia) influence soft-tissue attachment, and recognize the limits of the current evidence.
- Explain why repeated soft-tissue manipulation and second-stage surgery can offset the benefits of improved abutment surfaces.
- Outline socket preservation options for the esthetic zone, including the indications and steps of a vascularized pedicle combination epithelial-subepithelial tissue graft.
- Apply a tissue-focused treatment-planning sequence from extraction through final restoration.
